Questions

Can every broken file be recovered?

No. If the data is genuinely gone, nothing can invent it. Repair rebuilds structure around whatever survives, and tells you what could not be recovered.

What causes this kind of damage?

Interrupted downloads, failed transfers, and software that wrote the file incorrectly in the first place.
